Responsibilities

• Perform initial evaluations in accordance with APTA standards
• Develop and implement individualized plans of care
• Set and track patient goals and outcomes
• Provide evidence-based treatment to improve function and well-being
• Educate patients with customized home exercise programs
• Document progress and maintain compliance standards
• Communicate effectively with physicians, care teams, and insurance providers
• Manage discharge planning for optimal long-term results

Qualifications

• Licensed (or eligible) Physical Therapist in Michigan
• Strong commitment to patient-centered care
• Excellent communication and documentation skills
• New grads welcome!
