Physical Therapist - Full-Time

Conservative Care Occupational Health (CCOH) provides comprehensive occupational medicine and rehabilitation services designed to help employees recover efficiently, strengthen movement, and safely return to work. Our clinics focus on functional recovery, preventative care, and workplace-injury rehabilitation using evidence-based treatment and a whole-person approach.

We are looking for a skilled and compassionate Physical Therapist (PT) to join our Tennessee team. This role supports patient care across both our Lebanon and La Vergne clinics, helping us maintain continuity, coverage, and consistent treatment quality. Candidates must be comfortable with routine travel between locations as needed.

Position Summary:
The Physical Therapist will evaluate, plan, and implement treatment programs for patients recovering from workplace injuries or physical conditions that impact mobility and work performance. You’ll collaborate closely with providers, medical assistants, and rehabilitation staff to deliver high-quality therapeutic care while supporting safe return-to-work outcomes.

What You'll Do:

• Perform physical therapy evaluations and develop individualized treatment plans.
• Provide patient-centered rehabilitative care for musculoskeletal and work-related injuries.
• Monitor progress and adjust treatment plans based on response and functional outcomes.
• Document treatments, evaluations, and patient status within the EMR accurately and timely.
• Collaborate with clinic providers to support return-to-work pathways and functional capacity.
• Educate patients on movement safety, recovery strategies, and self-management techniques.
• Maintain a professional and empathetic approach in every patient interaction.
• Travel regularly between Lebanon and La Vergne clinics based on scheduling needs.

Clinic Schedule

Our Tennessee clinics operate Monday through Friday, 8:00 AM – 5:00 PM with no weekend or holiday hours.
This position is full-time, 40 hours per week.
