Physical Therapist -- Home Health -- Flexible Schedule!

Canyon Home Care and Hospice is seeking a dynamic Physical Therapist to join our Northern Colorado team!

As a Home Health Physical Therapist you will provide 1:1 care to patients in their homes (which may include Assisted Living or Independent Senior Living facilities.) You'll create an individualized plan of care, coordinate with the interdisciplinary team to ensure the patient's needs are met, and supervise care provided by the Physical Therapy Assistant, as applicable. A Physical Therapist (PT) administers physical therapy to patients on an intermittent basis in their place of residence. This is
performed in accordance with physician orders and plan of care under the direction and supervision of the Clinical Manager.
