Old Glory Home Health Care & Assistive Care is a clinician-owned, Medicare-certified, CHAP-accredited agency with Age-Friendly Care certification. We are seeking a Physical Therapist for part-time or PRN home health visits in communities east and northeast of Richmond.

What you’ll do
- Evaluate patients and develop individualized physical therapy plans of care.
- Provide skilled treatment to improve strength, balance, mobility, and functional independence.
- Educate patients and caregivers on home exercises, fall prevention, and safe movement.
- Collaborate with physicians, nurses, and other rehabilitation professionals to coordinate care.
- Supervise physical therapist assistants as assigned.
- Complete accurate, timely documentation in our electronic medical record.
Qualifications
- Active Virginia Physical Therapist license.
- Strong clinical assessment, communication, and organizational skills.
- Ability to work independently while collaborating with a multidisciplinary team.
- Reliable transportation for patient visits.
- Home health experience preferred.
