Contribute physical therapy recommendations to residents’ individualized plans of care.
Provide practical training, demonstrations, and competency-based coaching to direct-support employees.
Train employees on positioning, body mechanics, transfers, mechanical lifts, adaptive equipment, range-of-motion programs, and other physical therapy supports.
Observe the implementation of physical therapy programs and provide timely coaching or corrective guidance when needed.
Coordinate or support orthopedic, therapy, and other related specialist appointments, as assigned.
Complete accurate evaluations, progress notes, recommendations, and other required documentation within established timeframes.
Monitor residents’ progress and modify interventions when their needs, abilities, or goals change.
Promote resident dignity, choice, safety, independence, and meaningful participation in daily life.
