Job Summary

The Physical Therapist provides comprehensive physical therapy services to PACE participants, including assessment, care planning, intervention, education, and ongoing monitoring to support functional mobility and safety. This role collaborates closely with the Interdisciplinary Team (IDT) to ensure participant needs are identified, addressed, and incorporated into individualized care plans. The Physical Therapist also contributes to care coordination, documentation, quality initiatives, and participant and caregiver training within the PACE model of care.
