Position Overview
The Physical Therapist will be responsible for conducting thorough initial evaluations of patients, developing individualized treatment plans, and documenting findings accurately. This is an evaluation-only role; no ongoing treatment sessions are required.

Key Responsibilities:
- Conduct comprehensive physical therapy evaluations for patients in their homes or designated facilities.
- Develop individualized care and treatment plans based on evaluation findings.
- Communicate recommendations and care plans clearly to patients, families, and care teams.
- Complete and submit timely and accurate documentation in accordance with state and agency requirements.
Qualifications:
- Active and unrestricted Physical Therapist (PT) license in the state of Georgia.
- Previous experience in home health or outpatient settings preferred.
- Strong communication and documentation skills.
- Ability to work independently and manage time efficiently.
