Position Summary

We are seeking a compassionate and experienced Physical Therapist (PT) with specialized training in lymphedema management to join our Medicare-certified home health team. This role focuses on delivering high-quality, patient-centered care in the home setting, helping patients improve mobility, reduce swelling, manage chronic conditions, and enhance overall quality of life.

Key Responsibilities

• Perform comprehensive physical therapy evaluations in accordance with physician orders and Medicare guidelines

• Develop and implement individualized care plans focused on functional mobility and lymphedema management

• Monitor and document patient progress in EMR in compliance with Medicare and agency standards

• Educate patients and caregivers on home exercise programs, self-management techniques, and prevention strategies

• Collaborate with interdisciplinary team members including nursing, OT, ST, MSW, and physicians

• Participate in OASIS assessments and contribute to care coordination

• Ensure timely documentation and adherence to all regulatory requirements
