Choice Health at Home is seeking a compassionate PRN Physical Therapist to provide personalized care that improves physical function and quality of life.
What You’ll Do
Complete and document initial evaluations to identify patient needs and establish individualized treatment plans.
Provide therapeutic interventions to improve mobility, relieve pain, increase strength, and restore function.
Monitor patient progress and adjust treatment plans to support the best possible outcomes.
Educate patients and caregivers on exercises, safety, and strategies for continued recovery.
Collaborate with families and the healthcare team to ensure continuity of care.
Follow infection control practices, agency policies, and patient care standards.
Complete accurate documentation within company guidelines.
What You’ll Bring
Graduation from an accredited Physical Therapy program and successful completion of the state licensure exam.
A valid Physical Therapy license.
Current CPR certification.
A valid driver’s license and auto insurance.
One year of home health experience preferred.
Homecare Homebase experience preferred.
Physical Requirements
Ability to stand, walk, sit, climb, crawl, squat, or kneel for extended periods.
Ability to lift, carry, pull, or push up to 100 pounds.
