VMP Healthcare and Community Living is looking for a Physical Therapist to join our team!
You will plan, organize, develop and implement a progressive therapy program geared to improvement of resident's physical functions, psychological, social and economic well being.

Education Requirements:
- Bachelor's degree (B. A.) from four-year college or university; or one to two years related experience and/or training; or equivalent combination of education and experience.
- Physical Therapy License valid in the state of Wisconsin.
Job Responsibility Overview:
- Complete thorough assessments of residents upon receiving physical therapy orders.
- Identifies and prioritizes needs based on assessment and establishes appropriate plan with goals.
- Communicates with resident, family members, and staff accordingly.
- Implements interventions according to plan of care.
