Wabash Senior Living & Rehabilitation is seeking a compassionate and motivated Physical Therapist (PT) to join our in-house rehabilitation team.

What You’ll Do

• Evaluate residents and develop individualized plans of care

• Provide skilled physical therapy services based on each resident’s needs and goals

• Help residents improve strength, balance, mobility, and functional independence

• Monitor progress and adjust treatment plans as appropriate

• Collaborate with nursing, physicians, therapy staff, residents, and families

• Complete accurate and timely clinical documentation

• Maintain compliance with applicable state and federal requirements

Qualifications

• Current Illinois Physical Therapist (PT) license

• Strong communication and interpersonal skills

• Compassionate and resident-focused approach

• Ability to work effectively as part of an interdisciplinary team

• Skilled nursing or rehabilitation experience preferred, but not required

• New graduates are encouraged to apply!
