Physician Practice Manager
JOB PURPOSE\:
The Practice Manager is responsible for operational leadership, provider group management, performance accountability, and infrastructure oversight of the long-term care (LTC) and post-acute care physician and advanced practice provider (APP) group across Georgia.
This role ensures consistent clinical coverage, provider productivity, regulatory compliance, and alignment with skilled nursing facility (SNF) operations and value-based care models, including Medicare Advantage, Institutional Special Needs Plans (I-SNPs), population health initiatives, and quality performance programs.
The Practice Manager partners closely with Medical Directors, NPs, facility leadership, and corporate stakeholders to drive quality outcomes, operational excellence, provider engagement, financial performance, and growth across the Georgia market.
The ideal candidate will have demonstrated success leading multi-site provider operations, preferably within long-term care, post-acute care, skilled nursing, physician practice management, population health, or similarly complex healthcare environments, while maintaining a strong focus on operational excellence, provider engagement, and value-based care performance.
KEY RESPONSIBILITIES\:
Provider Group Leadership & Workforce Management
- Provide leadership and oversight for physician and APP operations across multiple long-term care, post-acute care, and skilled nursing settings.
- Lead provider recruitment, onboarding, retention, scheduling, coverage planning, productivity management, and performance accountability.
- Ensure appropriate provider deployment and coverage models based on census, payer mix, growth strategies, and operational needs.
- Develop and execute provider engagement and retention initiatives.
Productivity, Performance & KPI Management
- Establish, monitor, and improve key performance indicators (KPIs) related to provider productivity, encounter volume, documentation timeliness, quality outcomes, and operational efficiency.
- Utilize data analytics to identify trends, drive process improvement initiatives, and support decision-making.
- Implement coaching, corrective action plans, and performance improvement strategies as needed.
Quality, Population Health & Value-Based Care
- Drive performance related to rehospitalization reduction, preventive care gap closure, HEDIS/STAR measures, Medicare population management, and value-based care initiatives.
- Partner with providers and clinical leadership to support HCC/RAF coding accuracy, quality documentation, and optimal patient outcomes.
- Support operational initiatives involving Medicare Advantage, I-SNP, and other population health programs.
Financial Stewardship & Operational Excellence
- Support financial performance through effective provider utilization, productivity optimization, billing readiness, and documentation compliance.
- Analyze operational and financial data to identify opportunities for growth, efficiency, and resource optimization.
- Partner with operational, billing, coding, credentialing, and clinical teams to remove barriers impacting revenue cycle performance and provider effectiveness.
