Job Summary
Provides direct care to patients and serves as a consistent care coordinator through the continuum of care, assessing the physical, psychological, spiritual and social needs of the patient. Directs patients to the next appropriate service and tracks their progress. Assures that all patients receive appropriate evaluation, diagnosis, treatment, medical screening and medical evaluation whenever indicated, and that all medical patient care is appropriately documented in the medical record.

- Use positive communication in my tone of voice, body language, and choice of words.
- Respond to e-mails in a courteous manner, with-in twenty-four hours.

Qualifications
- Attending Required or MD Required
- 1-3 years experience preferred in a medical practice setting.
- Excellent Interpersonal and Communication Skills
- License required - Physician - New York State Board of Medicine Required
