Job Summary:
Guided by policies and procedures of the DAP Health Medical Clinics and under the direction of the Clinic Nursing Manager, the Prescription Coordinator will play a critical role in optimizing the clinic workflow to create a patient-centered environment while assisting Clinicians and staff with prescriptions and prior authorizations.
Job Status: Full time, Hourly, Non-exempt
Duties/Responsibilities:
- Assists with medication refills and prior authorizations requested by providers.
- Documents all patient encounters in the EHR system.
- Discontinues medications with the appropriate pharmacies.
- Works closely with Case Management, Home Care, and Community Health staff to ensure continuity of prescriptions.
- Complete all paperwork accurately and efficiently (i.e., prescription refills, new prescriptions, pre-authorization RX requests, etc.).
- Serves as patient liaison by assisting patients with questions and requests.
- Maintains unit operations by following policies and procedures; reporting needed changes.
- Maintains patient confidence and protects operations by keeping information confidential.
- Maintains safe and clean working environment by complying with procedures, rules, and regulations; adhering to infection-control policies and protocols.
- Maintains continuity among work teams by documenting and communicating actions, irregularities, and continuing needs.
- Conserves unit resources by using equipment and supplies as needed to accomplish job results.
- Contributes to team effort by accomplishing related results as needed.
- Requests and retrieves client files as needed.
- Answers telephone and relay messages.
- Performs other duties as assigned.
