Founded in 1987 through the merger of Health Associates of Provincetown (established in 1972) and the AIM Medical Center in Wellfleet (established in 1966). Primary Purpose of Position:

Reporting to the Site Medical Director, the Primary Care Clinician serves as a clinical leader within an interdisciplinary care team and provides comprehensive, continuous care to a panel of patients. The clinician collaborates with team members to coordinate care, promote health, manage chronic disease, and improve patient outcomes in alignment with OCHS’s mission and team-based care model.

Essential Functions:

  • Lead and collaborate within an assigned care team (including RNs, LPNs, Medical Assistants, Behavioral Health Clinicians, and Patient Access staff) to manage the health of a patient panel and champion shared accountability for outcomes.
  • Maintain an assigned patient panel and meet access and productivity expectations while balancing quality, continuity, and team-based care principles.
  • Participate in and/or lead daily team huddles to anticipate patient needs, identify care gaps, and coordinate workflow.
  • Provide clinical oversight, mentorship, and support to nursing and allied health staff to ensure team members work at the top of their licensure.
  • Collaborate with Integrated Behavioral Health providers and Community Health Workers to address social determinants of health and mental health needs.
  • Support same-day access, urgent care needs, and flexible scheduling strategies to meet patient demand.
  • Participate in patient care audits, performance improvement initiatives, population health activities, and value-based care efforts to improve clinical quality and outcomes.
  • Complete documentation in a timely manner consistent with organizational standards and regulatory requirements.
  • Collaborate with leadership and staff on care delivery improvements, workflows, and quality initiatives.
  • Participate in on-call responsibilities, weekend coverage, and skilled nursing facility coverage as assigned.
  • Maintain collaborative and professional relationships across clinical and administrative teams.
  • Participate in provider meetings, team meetings, and organizational initiatives as directed by the Site Medical Director or Chief Medical Officer.
  • Model professionalism, collaboration, and accountability consistent with OCHS values.
  • Perform other related duties and projects as assigned.
