Private Care Assistant [EL]

• Plan, coordinate, and monitor the health and well-being management of designated stakeholders.
• Provide individualized, person-centred care, including routine blood glucose and blood pressure monitoring.
• Support lifestyle and dietary management in accordance with individual health needs and appropriate care plans.
• Provide necessary health and physical care to stakeholders during domestic and international travel, as required.
• Monitor general well-being, maintain relevant health records, and communicate health-related observations to the appropriate healthcare professionals when necessary.
• Support continuity of care by ensuring that relevant health information, monitoring requirements, and care arrangements are appropriately coordinated during travel.
