Private Care Clinical Manager, Full-Time Days, 8a-4p, Atlantic Health

The Private Care Clinical Manager serves as the clinical and operational leader for Atlantic Private Care Services (APCS), Atlantic Health's private duty home care program. This position is responsible for overseeing daily clinical operations, ensuring regulatory compliance, maintaining quality and patient safety standards, managing clinical staff, supporting financial performance, and driving operational excellence. The Clinical Manager collaborates with physicians, hospital care management teams, community partners, and internal stakeholders to deliver exceptional patient-centered care while supporting the continued growth of Atlantic Private Care Services.


Principal Accountabilities:

1. Provides oversight and operational support for the coordination and delivery of high-quality patient care 24/7. Leads and facilitates efficient patient flow processes, including admissions, transfers, and discharges, ensuring seamless transitions and optimal resource utilization. Conducts team meetings to foster team engagement, uphold safety standards, and enhance the overall patient and family experience. Ensures payroll is accurate. Ensures appropriate staffing levels to meet patient acuity and volume, managing overtime, and aligning staffing with budgetary and productivity targets
2. Budget: Oversees the development, implementation, and management of annual clinical operating, capital, and staffing budgets, ensuring efficient resource utilization and alignment with financial objectives. Actively contributes to strategic planning efforts to support organizational priorities.
3. Performance Management: Leads the performance management process by conducting timely introductory and annual evaluations, ensuring alignment with organizational goals and individual development plans. Actively participates in strategic talent decisions, including recruitment and separation, while providing ongoing clinical and behavioral coaching to support staff excellence. Identifies and promotes professional development opportunities to foster continuous learning and career advancement. Applies progressive disciplinary practices consistently and equitably, addressing performance and conduct issues promptly with thorough documentation.
4. Service: Cultivates a staff-driven experience that prioritizes exceptional service for patients, physicians, colleagues, the community. Actively involves staff in advancing standards of patient care excellence. Embodies and promotes the organization’s vision, mission, values, and strategic priorities. Consistently evaluates the patient experience and implements focused action plans to enhance satisfaction and support the achievement of organizational objectives.
5. Growth: Serves as an effective representative of APCS and health system within the community, actively seeking opportunities to promote the organization.
6. Quality: Ensures the implementation of professional practice standards in alignment with organizational policies, evidence-based practice, and Atlantic Private Care Services' quality standards. Maintains compliance with all applicable federal, state, and local regulations, including New Jersey home care regulations, OSHA requirements, and Commission on Accreditation for Home Care (CAHC) accreditation standards. Oversees departmental quality performance, quality improvement initiatives, risk management activities, and ensures adherence to patient safety standards and regulatory requirements.
7. Professional Practice: Promotes shared decision making and professional autonomy through active participation in a professional governance structure. Maintains engagement with relevant professional organizations. Leads recruitment, retention and team engagement efforts, fostering a collaborative and
supportive departmental culture.
8. Develop, implement, and monitor the policies and procedures necessary to maintain the standards required by accrediting bodies and/or licensing agencies. Review and oversee all contracts with insurance companies, home care companies and state or county agencies. Assist, as needed, with arranging for the appropriate data to be gathered for insurance companies when requests are made for information to be sent to them for their records.
9. Assist agency staff with the implementation of computer programs and materials necessary to support all standards and provide appropriate management information, i.e. expiring authorization reports.
10. Serve as Acting Administrator and/or nursing resource person for office and field staff in the Director’s absence.

Required:

  • Bachelor's degree in Nursing
  • Current license as a Registered Nurse in the state of New Jersey.
  • Certification in BLS

Preferred:

  • Minimum of four (4) years of progressive leadership experience in private duty home care, home health, hospice, care management, or other community-based healthcare services.
  • Experience leading multidisciplinary clinical teams, managing regulatory compliance, quality improvement initiatives, staffing, and daily operations is strongly preferred.
  • Experience with home care regulations, accreditation standards, quality improvement, risk management, and state licensing requirements.
  • Experience building relationships with hospital case management teams, physicians, discharge planners, and community referral partners preferred.
  • Proficient with Microsoft Word, Excel, Outlook & PowerPoint
  • Master's degree in Nursing, Business, Healthcare, or Finance preferred
