Summary
Coordinates and analyzes data for the Montana VA Healthcare System’s Primary Care and Mental Health sites using the Patient Centered Management Module (PCMM), ensuring accurate resource allocation and process improvements.
This position, located at the Fort Harrison, MT VAMC, serves as a Program Analyst (PCMM) coordinator for all Primary Care, and Mental Health sites for the Montana VA Healthcare System.
Major duties of this position include but are not limited to... Provides guidance and recommendations to primary care service line, working in collaboration with the Group Practice Manager, to review and ensure accuracy of data entered in to the Patient Centered Management Module (PCMM) Makes recommendations to managers regarding allocation of resources, including potential uses of community care options Proposes and leads process improvement activities to support implementation and improvement of PCMM review, assignments, and education in collaboration with Primary Care clinical and administrative leadership Analyzes and interprets complex data. Researches and prepares comprehensive reports Collects, analyzes, and trends data sets. Makes recommendations to assist in the allocation of resources Develop tracking and reporting mechanisms to monitor process improvement and project management activities Review regulations and policies and provide input at the local level for interpretation and implementation, utilizing clinic practice management and communication strategies Attains and sustains knowledge of PCMM business rules and training materials to support continuous improvement processes Primary manager of PCMM Web data base. Ensures that team assignments are set up correctly, routinely monitors data, and modifies to support data integrity for PCMM, MHMM, and HBPCMM Acts as a subject matter expert for PCMM for the facility Facilitates communication between Group Practice Management office, Clinic Profile Management Team, and clinical leadership/admin support teams to correct deficiencies, implement station improvement activities, and provides updated guidance on clinic practice management Acts as a liaison between the Group Practice Manager and Service Line clinical and administrative leadership and serves as an active member of the Clinic Practice Management Work group/clinical operations team Promotion Potential: The selectee may be promoted to the full performance level without further competition when all regulatory, qualification, and performance requirements are met. Selection at a lower grade level does not guarantee promotion to the full performance level. Work Schedule: Monday - Friday, 8:00am - 4:30pm Telework: This position may be authorized for telework.
