Program Coordinator - Community Services
Job Description\:
• Coordinate training and implementation of Community Care Service Program (CCSP) at PruittHealth Home First, Inc. Case Management agencies
• Develop training courses, training guides, and process workflows for CCPS implementation
• Monitor referrals and coordinate communication with Area Agency on Aging (AAA)
• Manages and strategizes program development for CCSP to grow and meet operational goals and initiatives
• Demonstrates knowledge of contractual, legal, and regulatory requirements with Georgia’s Medicaid Elderly and Disabled Waiver Program (EDWP)
• Provides assistance and guidance to the PruittHealth Home First offices to ensure CCSP sites deliver services in accordance with contractual, legal, and regulation requirements
Job Duties\:
• Administrative
o Submit Programmatic Reports to the Department of Community Health (DCH)
o Generate and review Harmony Advance Reporting (HAR) reports monthly (SOP for Monthly Contacts, PA Information Report, Cost of Care Report, Provider List)
o Maintain trackers monthly (Supervisory, Scheduler, Eligibility Checks)
o Demonstrate knowledge of all DCH Policy Manuals
o Maintain and update PolicyTech documents and forms for CCSP
o Attend meetings (regional, Care Division, Provider Network Meetings, Train the Trainer)
o Present at Corporate Conferences regarding any CCSP updates
o Host monthly CCSP calls with all CCSP staff
o Assist with questions (clients, caregivers, providers, staff)
• Billing
o Notify offices of No Bill and Billable Late Lock-In
o Follow up with Medicaid status
o Follow up on Prior Authorization (PA) billing issues
o Set-up MOUs with AAAs to receive MFP payment
• Intake
o Follow up with Referrals
o Resolve PA issues
• Miscellaneous
o Manage user access in Microsoft Teams – Home First
o Manage user accesses in Harmony – Pruitt CCSP sites
o Upload pertinent documents from DCH’s Train the Trainer to Microsoft Teams – Home First
o Coordinate between ADRC and Pruitt Contracting team re\: annual MOU contracts for MFP assessments
