Program Manager - Host Home Services
Summary
Oversee a host-home program for individuals needing care, coordinating referrals, training mentors, conducting home visits, and ensuring compliance with service plans and documentation.
POSITON SUMMARY
As the Host Home Program Manager you are responsible for coordinating and overseeing services for individuals participating in the Program. This position serves as the primary agency contact for Host Home members and Host Home Mentors throughout the referral and admission process and during ongoing services. The Program Manager works closely with members, Host Home Mentors, Case Managers, guardians, MCOs, natural supports, and other members of the individual's care team to promote safe, person-centered, high-quality services. Responsibilities include coordinating referrals and admissions, participating in Person-Centered Service Plan (PCSP) development, conducting routine home visits and annual home inspections, monitoring service documentation and program requirements, supporting and training Host Home Mentors, and maintaining ongoing communication with the member's interdisciplinary team.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Member Case Management & Service Coordination
- Serve as the primary CHAM contact for Host Home Mentors regarding program expectations, member services, documentation, and day-to-day questions.
- Provide ongoing consultation, guidance, problem-solving, and support to Host Home Mentors.
- Assist Host Home Mentors in understanding the member's PCSP, goals, support needs, rights, preferences, and service expectations.
- Coordinate and assist with initial and ongoing training of Host Home Mentors.
- Identify additional training or support needs and coordinate appropriate resources.
- Provide guidance regarding documentation expectations, incident reporting, medication-related requirements, member rights, health and safety, and other applicable program requirements.
- Communicate concerns regarding Mentor performance, compliance, member safety, or program expectations to the Program Director and participate in corrective action or remediation planning as appropriate.
Home Visits, Monitoring & Quality Assurance
- Provide oversight of Host Home service documentation.
- Review service documentation for completeness, accuracy, timeliness, and consistency with the member's PCSP and authorized services.
- Monitor completion of required reports, data tracking, incident documentation, progress notes, and other program records.
- Ensure required member, Mentor, and Host Home documentation is maintained in accordance with agency policy and applicable HCBS requirements.
- Monitor required Mentor training, certifications, documentation, and other program qualifications and notify appropriate leadership of deficiencies.
- Review incident reports and other significant events involving assigned members and ensure appropriate notifications and follow-up occur.
Documentation & Compliance
- Assist with developing and improving Host Home procedures, forms, training materials, monitoring tools, and program resources.
- Participate in evaluating prospective Host Home Mentors and Host Home environments.
- Assist with matching prospective members and Host Home Mentors based on needs, preferences, personalities, lifestyle, accessibility, location, and other relevant considerations.
- Identify program barriers, trends, risks, and opportunities for improvement and communicate recommendations to the Program Director.
- Collaborate with other CHAM departments to ensure appropriate coordination of authorizations, billing, documentation, training, quality assurance, and member services.
