GENERAL DESCRIPTION: Coordinate and oversee assigned contracts and services within Goodwills Shared Living Provider program.

ESSENTIAL JOB FUNCTIONS: (Note: Qualified persons with disabilities are encouraged to apply. Reasonable accommodations will be made.)

  1. Promote the benefits of Shared Living services with potential participants and all stakeholders.
  2. Recruit high quality sub-contractors to meet the varied individual needs of current and future Shared Living participants.
  3. Ensure sub-contractor setting requirements are met prior to and during placement.
  4. Ensure sub-contractor training requirements are met prior to and during placement.
  5. Prepare for and actively participate in Individual Support Plan (ISP) development.
  6. Monitor the delivery and documentation of all contracted services, including habilitative teaching and support, medical support, and behavioral support.
  7. Complete and document monthly home visits.
  8. Take immediate action to correct service gaps and documentation errors.
  9. Maintain records as required, demonstrating competency in the use of case management software.
  10. Fulfill Rights Committee responsibilities.
  11. Fulfill Advisory Committee responsibilities.
  12. Monitor billing and budget plan performance and take action to meet requirements.
  13. Ensure compliance with policies, regulations, and standards.
  14. Perform work activities in conformance to Goodwill Values Statement which includes the values of Respect, Integrity, Compassion, Stewardship, Learning, Teamwork and Entrepreneurial Spirit.
  15. Immediately report any wrongdoing, unethical or improper conduct, fraud, fiscal mismanagement or misappropriation of funds, etc. (Refer to the Corporate Compliance Program Policy and Procedure for instructions on how to report.)
  16. Encourage and do not hinder in any way an employee making a report pertaining to Goodwills Corporate Compliance Program Policy and Procedure or making a report of work injury.
  17. Ensure no employee is retaliated against in any way as a result of making a report related to Goodwills Corporate Compliance Program Policy and Procedure or making a report of work injury.
  18. Welcome and embrace cultural diversity in all areas of Goodwill, i.e. customers, donors, volunteers, employees, or participants served.
  19. Complete other duties as assigned.
