Programme Coordinator (CREST - West Region)

Summary

Coordinates mental health outreach programs, plans events, and manages volunteers to support at-risk individuals and caregivers in Singapore’s West and Central regions.

The Community Outreach Team (CREST) serves as a mental health first-stop touchpoint. CREST promotes early identification of at-risk individuals and provides psychosocial intervention to support those with mental health needs and their caregivers in the community.

· Promote early recognition of signs and symptoms of mental health conditions and/or dementia.

· Improve mental health conditions and/or dementia literacy by organising targeted outreach events and activities to provide mental health and/or dementia information and education to residents and caregivers in the region.

· Provide psychosocial intervention such as techniques to reduce the risk of escalation of mental health needs.

· Provide collaborative and coordinated care for clients and caregivers through a case management approach.

· Work in partnership with health and community care partners and key stakeholders to co-manage and support persons with mental health conditions and dementia in the community.

Responsibilities

We are looking for a Programme Coordinator who will conduct programmes to suit the needs of the clients and coordinate these programmes.

As a Programme Coordinator, you will identify areas of improvements in these programmes, implement quality control and compliance measures and develop information materials for distribution to clients and volunteers. You will plan the manpower roster for each programme and guide volunteers in executing programme activities. You will coordinate with other organisations to execute programmes, manage communications and feedback from stakeholders and conduct outreach activities to other organisations and the community.

Programmes

· Work closely with key stakeholders to plan, organise and implement programmes and activities for residents in the West (Jurong West, Pioneer, Nanyang, Ayer-Rajah & West Coast) and Central (Potong Pasir & Braddell Heights) regions.

· Support the team on timely data collation and quarterly report submissions.

· Provide administrative, logistical, and site-related support to the team in running client-related and/or other CREST activities.

· Prepare the necessary resource material to be provided to clients and caregivers.

Ancillary Duties

· Support the operations of Viriya Mental Wellness Hub located at Kent Road.

· Planning and executing events within the organisation.

· Work with stakeholders for ad-hoc external programs.

· Any other duties, as assigned.
