Summary
Coordinate community care programmes for seniors and caregivers, engaging stakeholders and monitoring outcomes through data collection and reporting.
- Coordinate and support the planning, implementation, and evaluation of community care programmes for seniors and caregivers.
- Engage clients, caregivers, volunteers, and community partners to facilitate programme participation and service coordination.
- Monitor programme outcomes through data collection, reporting, and continuous quality improvement initiatives.
- Conduct ground visits and collaborate with internal and external stakeholders to enhance programme delivery and client experience.
- Support volunteer recruitment, onboarding, training, and coordination to ensure smooth programme operations.
- Manage programme administration, procurement, documentation, audits, grant support, and operational reporting.
Requirement
- Diploma/Degree in Healthcare, Social Work, Psychology, Public Health, Business, or related disciplines.
- Preferably 2 years of experience in healthcare or community care with strong stakeholder engagement skills.
- Proficient in Google Workspace and Microsoft Office applications.
