T.O.N.E. Home Health Services, Inc. is seeking a compassionate and experienced Psychiatric Nurse to join our growing clinical team in a field-based role providing in-home psychiatric nursing care to patients throughout Southeast Michigan.

This position is ideal for a nurse who enjoys direct patient interaction, independent clinical decision-making, and collaborating closely with interdisciplinary healthcare teams to improve behavioral health outcomes in the home setting.

We are looking for someone who is clinically strong, responsive, technology-comfortable, and passionate about delivering patient-centered psychiatric care where it matters most: in the patient’s home.

Schedule
Part-Time / Per Diem
Flexible scheduling based on patient census and visit availability

Compensation
Visit-Based Pay
Competitive per-visit rates based on experience

Responsibilities
Conduct psychiatric nursing visits in patient homes
Assess and monitor patients experiencing anxiety, depression, behavioral health concerns, dementia-related behaviors, and other psychiatric conditions
Perform nursing assessments and symptom monitoring
Provide medication education and monitor treatment compliance
Collaborate with physicians, nurse practitioners, therapists, social workers, and interdisciplinary team members
Educate patients and families regarding behavioral health care plans
Identify changes in patient condition and communicate concerns promptly
Complete timely and accurate documentation within Kinnser/WellSky EMR
Support continuity of care through strong communication and follow-up
Participate in interdisciplinary care planning as needed

Qualifications
Current Michigan RN license
Psychiatric nursing experience required
Minimum 2 years of nursing experience required
Home health or community-based care experience strongly preferred
Strong clinical assessment and critical thinking skills
Comfortable with technology and EMR documentation
Excellent communication and patient education skills
Valid driver’s license, reliable transportation, and current auto insurance
