PT RN - Home Care
POSITION SUMMARY: Under the direct supervision of the Home Care Supervisor and Manager, provides skilled nursing assessments to participants. This position is responsible for working directly with the Interdisciplinary Team (IDT), participant, and his or her representative to achieve the goals of the care plan. Communicates to the family or care giver and provides appropriate patient education.

RESPONSIBILITIES AND DUTIES:
The principal duties of this position include, but are not limited to, the following:
- Must maintain calm and professional demeanor.
- Documents assessments, care, and care coordination in the EHR.
- Completes regulatory assessments of participants (annual, and semi-annual), as well as initial assessments.
- Provides medication reconciliation and communicates with providers, facilities, and the pharmacy during assessments.
- Communicates effectively to the IDT and to family and caregivers, including providing instructions regarding medications and treatment.
- Performs skilled nursing care procedures such as wound care, fingerstick for blood glucose/INR, measures for and applies compression garments, and other care as assigned and according to licensure regulations.
- Provides patient education related to medications, inhalers/nebulizer treatments, and point of care testing such as blood glucose and INR.
- Attends and participates in Interdisciplinary (IDT) team meetings as assigned.
- Prepares nursing problems/interventions and evaluations for care plans and presents at IDT team meetings as assigned.
- Performs phlebotomy functions for participants when necessary. Identifies and labels specimens appropriately. Handles potentially infectious specimens with appropriate biohazard precautions, and practices universal precautions.
- Observes, records, and reports participants condition and reaction to drugs and treatments to medical providers, using SBAR format.
- Follows all Rocky Mountain Policies and Procedures and OSHA safety guidelines. Maintains the confidentiality of all company procedures, results, and information about participants, clients, or families.
- Participates in continuing education classes and any required staff and training meetings. Maintains professional affiliations and any required certifications.
- Performs other duties as required or requested by supervisor in a positive and helpful manner to ensure a smooth-running work area.
