PTA | Physical Therapy Assistant | PACE Program
***Attention Physical Therapy Assistants!***
Are you fed up with trying to work at 95% productivity? Tired of Administration mandating your caseload? Frustrated with not being able to fully use your professional judgment when making clinical decisions? Burnout rates in Physical Therapy are climbing, driven by high patient volumes, excessive documentation, and administrative burdens.
Your solution is PACE! This community health model of care is how health care should be delivered. Our all-inclusive approach allows you, the Physical Therapy Assistant, to provide the necessary care using your judgment and decision-making skills.
Midland Care Connection
Midland Care Connection is a not-for-profit, community-based organization dedicated to helping older adults and seriously ill individuals stay independent and supported in their homes. Established in 1978, we offer a full continuum of care, including hospice, palliative care, home health, grief and loss counseling, and our signature PACE (Program of All-Inclusive Care for the Elderly) program. Our mission centers on "meeting individual needs through innovative care solutions," and we serve people across multiple counties in northeast Kansas.
PACE Program
The PACE (Program of All-Inclusive Care for the Elderly) program is designed to help older adults remain safely in their homes rather than entering nursing facilities. It provides complete medical care, rehabilitation, medications, transportation, social services, and supportive therapies—all coordinated by an interdisciplinary care team. Participants receive personalized care plans tailored to their unique needs, along with access to a PACE center for social activities, meals, and clinical services. By integrating medical and supportive care under one program, PACE helps seniors maintain independence, improve quality of life, and continue living in their communities with dignity.
Physical Therapist Assistant
A Physical Therapist Assistant (PTA) at Midland Care’s PACE program works under the direction of a licensed physical therapist to help older adults maintain or improve their strength, balance, mobility, and independence. The PTA provides therapeutic exercises and functional activities such as walking and transfers, helps participants use mobility equipment safely, and provides education on exercises and fall prevention. PTAs may work with participants in the PACE center, their homes, or other community settings. They also monitor participants’ progress, document treatment, and communicate changes or concerns with the physical therapist and interdisciplinary care team. Overall, the PTA’s role is to help PACE participants remain safe, active, and as independent as possible in their daily lives.
