Public Health Project Coordinator (Hepatitis C Investigator and Linkage to Care Coordinator)

Under the general supervision of the Communicable Disease Manager, the Public Health Project Coordinator (Hepatitis C Investigator and Linkage to Care Coordinator) investigates reported cases of Hepatitis C Virus and provides Hepatitis C education, testing, and referral services. This position requires monitoring patients, answering questions, and providing support to enhance patient engagement andtreatment of Hepatitis C within the community.

Examples of Duties

  • Provides health information to the public and makes referrals to appropriate agencies through individual phones, and personal contacts
  • Refers patients for follow-up confirmatory testing and treatment as needed
  • Listens to client concerns about diagnoses and testing with patience and care
  • Participates in the design and implementation of Hepatitis C program to meet the needs of identified populations.
  • Maintains detailed, accurate, and confidential records and documentation in health surveillance systems
  • Investigates reported cases of Hepatitis C by interviewing infected individuals to determine the potential source of illness.
  • Delivers compassionate and non-judgmental customer services to educate individuals and at-risk populations by telephone ,mail and in clinic settings
  • Provides consultation and direct services to defined at-risk populations
  • Makes referrals to health clinics, encouraging individuals to obtain medical evaluation and following up to ensure they receive appropriate treatment
  • Assist with monitoring the quality of the Hepatitis C disease surveillance data
  • Monitors patient testing and treatment progress to ensure patients receive education and complete treatment for Hepatitis C
  • Tests patients for Hepatitis C as needed, and provides pre and post-testing education
  • Provide community education and health promotion on hepatitis C prevention, transmission, testing, treatment, and available resources to increase awareness and support linkage to care.
  • Assist with community outreach and field-based activities, including health fairs, screening events, and other community initiatives, to promote hepatitis C prevention, testing, and access to treatment and supportive services.
  • Other duties assigned by supervisor
  • Certification in Incident Command Structure Crisis Management, and participation in Public Health Emergency
  • Preparedness activities in real or simulated emergencies

Minimum Qualifications

Bachelor's or Master's degree Public Health, Business Administration, Health Science Administration or other related field, with two years experience in community engagement, community organizing, and project coordination and implementation.

Equivalent combinations of education and experience may be substituted to meet the education and experience requirement of this position.

Preferred Qualifications

  • A Bachelor’s Degree in nursing (BSN) with a Registered Nursing License in the State of Michigan
