Avondale House is a 501(c)(3) not-for-profit agency that provides services to individuals with autism, a day program for adults, where clients receive training in daily living skills and pre-vocational activities, and four residential homes for individuals unable to live in their own home. Avondale House has been serving individuals with autism since 1976.

Job Duties:
· Ensures effectiveness of individuals with intellectual development disabilities active treatment program.
· Develops, implements, and monitors each resident’s Individual Program Plan (IPP).
· Coordinates, monitors, documents, and follows up on all client programs, individualized plans, and services, correcting inconsistencies and reviewing for any adjustments that may be required.
· Conducts annual and quarterly client reviews.
· Completes ICAPS.
· Completes E-scores for new admissions and annually for all residents to successfully rate each client during fire drills.
· Maintains Emergency Preparedness Plan.
· Reviews monthly data sheets.
· Visits off-site day programs.
· Facilitates ongoing training for staff on state and federal regulations, behavior programs and data collection.
· Completes ID/RC and Living Option via SASS state website.
· Completes critical incident data for each residential home and inquiry forms monthly via Care website.
· Completes therapeutic passes via THMP state website.
· Completes required Medicaid recertification forms as needed.
· Ensures quality of program and protection of client’s rights, investigates incidents as needed.
· Ensures each home is in compliance with organization’s standard of cleanliness.
· Monitors for ongoing compliance with agency directives, licensing standards, and policies and procedures.
· Ensures compliance with federal, state, and organizational policies and regulations.
· Conducts initial onboarding and ongoing training programs for staff members.
· Ensures the Implementation of IPP’s and BSP’s for individuals in residential facilities.
· Ensures the safety and security of residents and staff.
· Implements emergency procedures when necessary.
· Participates in team program planning by attending quarterly and annual IDT meetings.
· Ensures timely completion and entry of house documentation into TaskMaster Pro system as required.
· Collaborates with other departments and external agencies for comprehensive care.
· Conducts weekly supervisory monitoring visits in the homes during the hours that the clients are present.
