Location: Orland Park, Il & Home Based Services
Schedule: Full-Time | Monday–Friday | Days
Local Travel: Required
Vehicle: Must have reliable personal vehicle for work-related travel
Pay Rate: $23-$25/hour

Looking for a position where you can truly make a difference? CTF Illinois is looking for a dedicated and organized QIDP / Case Manager – SDA to support individuals with intellectual and developmental disabilities through our Home Based Services program. In this role, you’ll have the opportunity to build meaningful relationships, help individuals work toward their goals, and coordinate the services and supports they need to live as independently as possible. What You’ll Do

  • Coordinate and implement Individual Service Plans (ISPs) based on each individual’s goals, needs, preferences, and recommendations from medical and professional consultants.
  • Conduct regular home visits and maintain consistent contact with individuals receiving services.
  • Assist Personal Support Workers (PSWs) with implementing ISPs, budgeting, shopping, and other money-management needs.
  • Submit required DHS documentation accurately and in a timely manner.
  • Monitor each individual’s progress through monthly reviews and maintain accurate personal history documentation.
  • Coordinate services and communicate with other departments, including Nursing, CILA, and Day Programs.
  • Complete required continuing education and training to maintain QIDP qualifications.
