Summary
Provides on-site crisis intervention, case management, and recovery support for individuals with chronic mental illness in Collin County, including home visits, school outreach, and community-based follow-ups under federal/state guidelines.
This position is team based to perform face to face crisis assessments, crisis intervention, crisis follow up and relapse prevention services for individuals with chronic mental illness within Collin County. Services will be provided at the individual’s place of residence, schools, and other safe community-based locations 24 hours per day, 365 days per year, including holidays. Services will be provided to adults/adolescents /children who are physically located in Collin County at the time of the crisis. Staff coordinates, monitors, provides crisis assessments, recovery planning, serves as an advocate and liaison on behalf of the individual served and follows up with individuals for 90-day period. Staff works within federal, state, and local guidelines to provide routine case management, skills training, psychosocial rehabilitation as well as community resource education within 90-day period.
Hiring for 3-12 hour shifts:
- WED, THURS, FRI 7AM-7AM
- FRI, SAT, & SUN 7PM-7AM
