About the Role:

The Quality Improvement Associate supports the Quality team in advancing clinical performance and patient care outcomes. Reporting to the Manager of Quality Improvement, this role is responsible for monitoring key quality measures, identifying care gaps, and coordinating outreach efforts to ensure patients receive appropriate preventive services and chronic disease management. The Associate collaborates closely with providers, care teams, and external partners to drive data-informed improvements and enhance patient outcomes.

Key Responsibilities:

As a Quality Improvement Associate, your responsibilities will include:

  • Monitor and report on key clinical quality measures (e.g., cancer screenings, depression, diabetes care).
  • Develop and manage patient registries to identify and track care gaps.
  • Conduct patient outreach to support preventive care and chronic disease management.
  • Participate in payer case conferences and complete follow-up activities.
  • Prepare and distribute quality performance reports to care teams.
  • Support reporting, dashboards, and presentations for internal and external stakeholders.
  • Assist with documentation, meeting minutes, and quality improvement initiatives.
  • Address patient complaints related to quality of care, as needed.

Work Schedule:
This full-time position is Monday - Friday, 9am-6pm. Occasional evenings.
