- Responsible for managing the data on the DCC Clinical
dashboard.
- Responsible to focus on the following dashboard reports:
- Hospital Report:To be checked daily. Any patients not entered,
the clinic manager is contacted and asked to enter any missing
information. After 7 days of hospitalisation the clinic must
update on the patient’s status as well as updating the dashboard report.
- Modality Option Education Report: Any patients that have been
listed for 6 months will be archived.
- Report will be cleaned up at the beginning of every month.
- Discharge/Modality Change/Transfer Report: Once patients are
discharged verification will be made that they are discharged from
GAIA and Ascend as well. If PD, verification of patient
removal in Share source is made. If HHD, verification of
removal from NxStage is made. Discharge report pulled from GAIA
and compared to dashboard to ensure all patients are listed.
- Missed Treatment Report: Integrity of information is compared
with EMR. Follow up is done on patients who have
Transportation issues by contacting the social worker for
assistance.
- Dialysis Infection Tracking Report: Comparison to Ascend and
GAIA reports will be utilised to ensure data on dashboard is accurate.
- Other reports will be added as needed.

Requirements
Work
Experience:
- 3 years of
experience in patient coordination or healthcare
setting
- Strong
communication and motivational skills.
Education:
- Bachelor’s or
Master’s degree in Healthcare
Administration, Nursing, Public Health, or related
field.
Skills:
- Must be a self-starter who can work independently yet function
in a highly collaborative, fast-paced team environment.
- Ability to multi-task, work under pressure and meet deadlines effectively
- Excellent written and oral communication skills
- Strong data entry, organizational and interpersonal skills
- High integrity, team player with patient service focus and
flexibility
