Re-Entry Specialist - Mercer

Scope of Duties:

1. Interact with all clients, staff, and outside contacts with respect and courtesy.
2. Maintain client and staff confidentiality in compliance with administrative policies and procedures.
3. Recruit clients to participate in program including visiting the county jail to interview prospective clients.
4. Complete intake interviews with clients and secure program participation agreements.
5. Assist clients apply for social entitlements such as GA, TANF, etc. when appropriate.
6. Link homeless clients to housing
7. Attend and participate in regularly scheduled staff meetings, supervisory conferences, and training sessions.
8. Monitor and document client progress
9. Assess client needs and work with client to develop goals
10. Update service plan with client input.
11. Provide motivational support to clients in program and maintain support throughout participation.
12. Schedule and facilitate meetings with clients as required.
13. Identify and/or develop resources and link clients to needed services.
14. Facilitate scheduled workshops/programs/groups addressing client's needs.
15. Develop appropriate discharge plan on all assigned clients.
16. Participate in required internal/external meetings
17. Complete all required reports, data collection and other documentation.
18. Other duties as assigned.

The employees in this position must have a Bachelors Degree preferably in a Human Services related field or at least 5 years relevant experience. Transferable experience is preferred. Must possess effective written and verbal communication skills, and an ability to interact appropriately with clientele. Basic computer skills are preferred. Must pass a criminal background check.
