Referral Coordinator RN- at Providence Queen of the Valley Medical Center in Napa, CA. This position is Per Diem and will work 8-hour Day shifts.

Under the general supervision/direction of the Acute Rehabilitation Nurse Manager, and in collaboration with the treatment team and external entities, the Rehabilitation Admissions Nurse shall monitor, coordinate and facilitate the interdisciplinary plan of care, effective clinical resource management, discharge planning, and continuation of post acute care needs of patients assigned. The purpose of the Rehabilitation Referral Management program is to promote quality of care, cost effective patient outcomes, efficient resource utilization, while meeting the expectations of patient, family, referring physicians and agencies, and payer expectations.

Required Qualifications

  • Upon hire: California Registered Nurse License
  • Upon hire: National Provider BLS - American Heart Association
  • 2 years of Rehabilitation Nursing.

Preferred qualifications:

  • Bachelor's Degree in Nursing.
  • Upon hire: Certified Rehab Registered Nurse (CRRN)
