Position Details:
- Main Office location: Londonderry, NH
- Other Office locations to manage: Concord, NH, Exeter, NH and York, ME
- Service Area: All of New Hampshire and Maine operations
- Job Type: Full Time
- Schedule: Monday - Friday, with regular travel across the region's four Centers
Responsibilities:
- Support and guide the Clinical Managers and Clinical Supervisors across four Centers, building strong working relationships that help them run their teams effectively.
- Step in as acting Clinical Manager to keep a Center running smoothly in the event of a vacancy in that role.
- Oversee clinical and administrative operations across all four Centers, ensuring referrals, admissions, visits, documentation, and physician communication meet Pathways' standards.
- Serve as a liaison between the Vice President of Clinical Operations/COO and Center leadership, helping communicate new initiatives and ensuring issues are resolved at the Center level in a timely manner.
- Support Clinical Managers and Supervisors in leading effective Daily Operations Briefings and keeping their teams engaged and informed.
- Assist in selecting, onboarding, and setting up new Clinical Managers and Clinical Supervisors for success, and ensure new team members across Centers complete competency evaluations within 90 days of hire.
- Provide feedback and performance evaluations (30-day, 90-day, and annual) to the Clinical Managers and Supervisors who report to them.
- Monitor quality and safety across the region, including incident reporting, infection control, and patient feedback, and support performance improvement initiatives.
- Partner with Human Resources and Center leadership to forecast staffing needs and support productivity, appropriate use of per diem staff, and accurate billing across all four Centers.
- Play a lead role in the opening of new Centers, providing hands-on leadership during the growth phase.
- Maintain clinical competency and provide direct or joint patient visits as needed to support Centers and their teams.
Qualifications:
- Active RN, PT, or OT license in the state(s) of practice.
- Nursing, physical therapy, or occupational therapy degree from an accredited school.
- Current BLS CPR certification with in-person verification of competency.
- Minimum of five (5) years of experience as a practicing nurse, physical therapist, or occupational therapist; home health experience preferred.
- At least two (2) years of experience in a supervisory role.
- Prior experience as a Clinical Manager or in a comparable home health leadership role strongly preferred.
- Current driver's license and reliable vehicle with state-required insurance; ability to travel regularly among the region's four Centers and to patients' homes as needed.
- Excellent verbal and written communication, interpersonal, organizational, and problem-solving skills.
- Ability to exemplify Pathways' TRUST values and foster a positive, purposeful culture across multiple teams.
- Comfortable with computers, tablets, smartphones, EMR systems, and learning new technologies; Homecare Homebase experience preferred.
