The Registered Dietitian (RD) is responsible for assessing, planning, implementing, and monitoring nutrition care for residents in accordance with federal, state, and facility regulations. This role collaborates with the interdisciplinary team to promote optimal nutritional health, enhance quality of life, and support positive clinical outcomes.

Reporting: This position reports directly to the Administrator and works collaboratively with the Director of Nursing Services (DNS), Medical Director, and Interdisciplinary Care Team.

Key Responsibilities

Conduct comprehensive nutrition assessments for residents upon admission and as required by regulation or clinical need.

Develop, implement, and monitor individualized nutrition care plans.

Evaluate residents for nutritional risk, weight changes, wounds, swallowing concerns, and therapeutic diet needs.

Monitor and document residents’ nutritional status and progress in accordance with facility policies and regulatory requirements.

Participate in care plan meetings and collaborate with physicians, nursing staff, therapy, and other interdisciplinary team members.

Provide nutrition education and counseling to residents, families, and staff as appropriate.

Review and approve menus to ensure nutritional adequacy and compliance with regulatory standards.

Monitor food service operations and provide guidance regarding therapeutic diets, food preferences, and nutrition-related concerns.

Support facility quality improvement initiatives and maintain compliance with state, federal, and CMS regulations.

Maintain accurate, timely, and complete documentation in the electronic medical record.
