Registered Nurse

AlphaCare Home Health Agency is looking for a compassionate and dependable Registered Nurse (RN) to join our team. In this role, you will provide skilled nursing care to patients in their homes while serving as a Case Manager for an assigned caseload. You will play an important role in helping patients maintain their health, independence, and quality of life through individualized care and ongoing support.
We are seeking a clinician who is confident working independently, communicates effectively with patients and care teams, and is committed to delivering high-quality, patient-centered care.
Responsibilities Include:
- Perform patient assessments and develop individualized care plans
- Provide skilled nursing care and implement physician orders
- Monitor patient progress and communicate changes appropriately
- Educate patients and caregivers on medications, treatments, disease management, and safety
- Coordinate care with physicians and interdisciplinary team members
- Complete clinical documentation accurately and within agency timelines
- Supervise LPNs and Home Health Aides as needed
- Prioritize visits based on patient needs and clinical status
Requirements
- Current Massachusetts RN license
- Graduate of an accredited nursing program
- Minimum one (1) year of nursing experience required
- Home Health experience preferred
- Current BLS certification
- Strong clinical, organizational, and communication skills
