Implement and monitor individualized care plans while documenting patient conditions to support high-quality care outcomes.
Coordinate care across multidisciplinary healthcare teams to ensure seamless treatment and holistic patient care.
Execute clinical orders efficiently to deliver accurate and timely care for every patient.
Assess and align patient discharge needs with care team members to ensure smooth and safe transitions to continuing care.
- Required Credential(s):
- BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date. American Heart Association or American Red Cross accepted.
- Registered Nurse obtained prior to hire date or job transfer date.
- Diploma from an accredited school/college of nursing required. OR Required professional licensure at time of hire.
