- Conduct comprehensive biopsychosocial assessments and deliver direct nursing care to guide patients through individualized treatment journeys.
- Design, implement and adapt personalized care plans that align with multidisciplinary clinical goals and evolving patient needs.
- Partner with interdisciplinary care teams in case conferences to refine treatment objectives and ensure seamless, coordinated care.
- Facilitate therapeutic group sessions and clinical interventions, continuously evaluating patient progress to drive positive health outcomes.
Licensure / Certification / Registration:
- BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
- Registered Nurse credentialed from the Illinois Board of Nursing obtained prior to hire date or job transfer date required.
Education:
- Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.
Additional Preferences:
- A minimum of one year of registered nurse experience preferably within the field of behavioral health is a must.
