- Assess & Understand: Conduct initial and ongoing bio-psychosocial assessments using trusted nursing standards, creating a personalized care plan that aligns seamlessly with each patient's master treatment plan.
- Collaborate & Connect: Join supportive treatment team conferences to help set, adapt, and refine goals, objectives, and care strategies tailored to each patient's unique background and needs.
- Support & Facilitate: Bring nursing care plans to life for your assigned patients, and lead or co-lead engaging group therapy sessions that foster healing and growth.
- Evaluate & Evolve: Walk alongside patients to evaluate how they respond to care, teaming up with colleagues to update plans and interventions whenever a patient's journey takes a new step.
- Required Credential(s):
- BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date. American Heart Association or American Red Cross accepted.
- Registered Nurse obtained prior to hire date or job transfer date.
- Approved Local Exception. Approved Local Exception for TN to add NRP and STABLE. or For use with unique Union job requirements only. Remove if not for Union.
- Drivers License.
- Diploma from an accredited school/college of nursing required. OR Required professional licensure at time of hire.
