Registered Nurse Case Manager

The Case Management Registered Nurse (CM RN) is responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. The Social Work and Nursing Case Management Service Line is a multidisciplinary team that addresses the healthcare needs of veterans. The incumbent will provide nursing support to inpatient medicine and specialty care teams, aiding in the coordination of post-discharge services.

Additionally, the incumbent provides nursing support to PACT teams, specialty clinics, and community-based outpatient clinics. Supports and lead in the provision of appropriate care for Veterans ranging from 18 through the geriatric years. Evaluates care and outcomes to ensure timely and appropriate provision of services. Influences care outcomes by collaborating with members of the interdisciplinary team and executes position responsibilities that demonstrate leadership, experience, and creative approaches to the management of complex client care. Coordinates care with multiple providers across all levels and sites of care. Addresses psychosocial, as well as nursing and medical needs of patients and their families/caregivers, through participation in interdisciplinary patient care management practice.
