Registered Nurse Home Health

• Department: Home Health Therapeutic Services

• Schedule: Day shift | Part-time | 20 hours per week

• Deliver compassionate direct care — including triage, blood draws and wound management — to improve patient comfort and maintain high-quality clinical records.

• Design personalized care plans alongside a collaborative medical team to ensure every patient receives targeted and effective treatment.

• Coordinate seamless transitions across care disciplines and support discharge planning to safeguard long-term patient recovery.

• Empower patients, caregivers and staff with essential health education during interactive care meetings.

• Travel to local homes and care facilities as part of a flexible 20-hour-a-week schedule to provide critical support exactly where patients need it most.

Licensure / Certification / Registration:

• BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.

• Drivers License credentialed from the Illinois Department of Motor Vehicles (IL-DMV) obtained prior to hire date or job transfer date required.

• Registered Nurse credentialed from the Illinois Board of Nursing obtained prior to hire date or job transfer date required.

Education:

• Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.

Additional Preferences:

• 2 years of hospital or home health experience is a must.
