- Lead the implementation of personalized care plans, continuously monitoring patient progress to ensure every individual receives the right intervention at the right time.
- Coordinate a multidisciplinary team of specialists to provide a seamless, holistic care experience that prioritizes the patient’s clinical and emotional needs.
- Translate complex physician orders into compassionate bedside actions, maintaining the highest standards of safety and clinical excellence.
- Advocate for patients throughout their recovery journey, documenting every step of the care process to ensure clear communication and high-quality outcomes.
- Facilitate comprehensive discharge planning with the healthcare team to ensure patients and their families feel confident and supported as they transition home.
Licensure / Certification / Registration:
- BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
- Registered Nurse credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date required. Licensure from the Wisconsin Board of Nursing OR current home state license if considered multi-state/Compact State.
Education:
- Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.
