• Lead bedside care by implementing and adapting personalized care plans that prioritize patient safety and rapid recovery.
• Coordinate multidisciplinary teams to ensure a seamless, holistic care experience where every specialist aligns with the patient's goals.
• Monitor and interpret patient conditions in real-time, acting as the critical link between the patient and medical staff to trigger timely interventions.
• Advocate for patients and families by managing the discharge process, ensuring a smooth and confident transition from hospital to home.

Licensure / Certification / Registration

• Required Credential(s):
  • Registered Nurse credentialed from the Indiana Board of Nursing obtained prior to hire date or job transfer date.
  • One or more of the following:
    • BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date. American Heart Association or American Red Cross accepted.
    • Instructor specializing in Basic Life Support obtained within 1 Month (30 days) of hire date or job transfer date. American Heart Association or American Red Cross accepted.

Education

• Diploma from an accredited school/college of nursing required. OR Required professional licensure at time of hire.
