- Executes individualized patient care plans, tracking status, documenting clinical changes, and promptly communicating updates to the care team.
- Acts as the primary liaison across multidisciplinary healthcare teams to ensure seamless, well-aligned treatment and communication.
- Accurately notes and carries out physician and nursing orders, adhering to strict clinical protocols and patient safety guidelines.
- Evaluates discharge readiness and collaborates with clinicians, social services, and families to design smooth, safe transitions of care.
- Required Credential(s):
- BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date. American Heart Association or American Red Cross accepted.
- Registered Nurse credentialed from the Texas Board of Nursing obtained prior to hire date or job transfer date.
- Pediatric Advanced Life Support. American Heart Association or American Red Cross accepted.
- Diploma from an accredited school/college of nursing required. OR Required professional licensure at time of hire.
