Southeastern Renal Dialysis is seeking an experienced Registered Nurse (RN) – Clinical Team Lead to oversee and coordinate our Home Dialysis Program for patients with End Stage Kidney Disease (ESKD).

This leadership role is responsible for delivering high-quality patient care while supervising and supporting RNs, LPNs, Patient Care Technicians, and Home Therapy Care Team Assistants. The Clinical Team Lead ensures clinical excellence, regulatory compliance, staff development, and strong patient outcomes.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

• Assess, plan, implement, and evaluate care for assigned home dialysis patients, providing ongoing clinical oversight
• Train and educate patients and care partners on safe, effective home dialysis therapy and treatment modalities, including transplant options
• Monitor treatment data, laboratory results, and patient response; adjust care plans and communicate updates to physicians and the interdisciplinary team
• Administer medications as prescribed and respond appropriately to clinical changes or emergencies
• Ensure accurate lab collection, processing, and documentation in compliance with regulatory standards
• Lead and oversee the home dialysis training program, ensuring high standards of patient competency and safety
• Supervise, mentor, and support clinical staff; assist with orientation and ongoing training
• Serve as a clinical resource and act as Nurse in Charge when assigned
• Ensure compliance with state, federal, and CMS regulations
• Support and drive quality improvement and infection control initiatives
• Participate in scheduling and rotating on-call coverage, including nights, weekends, and holidays as required
