At Auburn Crest Primary & Palliative Care, we bring healthcare directly to seniors living in assisted living and memory care communities.

We're looking for an experienced RN to provide clinical oversight for a designated region, supporting residents, mentoring LPNs, and partnering with providers to deliver exceptional care.

This is a leadership-focused nursing role that allows you to remain connected to patients while enjoying greater flexibility and autonomy than traditional hospital nursing.

What You'll Do

• Provide clinical oversight and support to a team of LPNs
• Perform resident assessments and clinical reviews
• Partner with Nurse Practitioners on care planning and interventions
• Educate community staff and caregivers
• Support families through changes in condition and care transitions
• Help improve outcomes and reduce unnecessary hospitalizations
• Build strong relationships with community leadership

Qualifications

• Active Idaho RN license
• CPR/BLS certification
• Strong assessment and leadership skills
• Experience in senior care, home health, hospice, assisted living, memory care, or care management preferred
• Reliable transportation for local travel
