Registered Nurse RN for Home Health--Flexible hours! No On-Call!

We are seeking RN Case Managers to provide 1:1 skilled care to patients in their homes. You will assess your patient, develop and execute a client-centered plan of care in collaboration with the interdisciplinary team. Schedule is flexible with no weekend requirements and consistent support from clinical manager team. We are a Medicare certified agency providing care to a wide variety of clinical needs, ages ranging from 18-100+. QUALIFICATIONS:

1. Graduate of an approved school of professional nursing and currently licensed in Colorado.
2. One (1) year nursing experience, preferred.
3. Acceptance of philosophy and goals of this Agency.
4. Ability to exercise initiative and independent judgment.
