Your Role as a Registered Nurse – Community Aged Care

Location: Travel required across Chermside to Beaudesert

As a Registered Nurse with WMQ’s Community Aged Care team, you will deliver high-quality, person-centred clinical care that supports customers to remain independent, safe, and connected within their own homes and communities. Working across the Chermside to Beaudesert region, you will provide holistic nursing assessments, care planning, clinical interventions, education and support, while promoting dignity, wellbeing and quality of life.

Using a biopsychosocial approach and evidence-based practice, you will work collaboratively with customers, families and multidisciplinary teams to achieve the best possible health outcomes.

What We’re Looking For

  • Current registration with the Australian Health Practitioner Regulation Agency (AHPRA) as a Registered Nurse.

  • Demonstrated clinical nursing experience with knowledge of contemporary nursing practice and care documentation.

  • Ability to conduct comprehensive assessments, develop care plans and provide person-centred nursing interventions.

  • Strong understanding of holistic, biopsychosocial models of care and the ability to support customer independence and wellbeing.

  • Ability to lead, coach and support Personal Carers, Assistants in Nursing and Enrolled Nurses.

  • Sound understanding, or ability to quickly acquire knowledge, of aged care legislation, standards and accreditation requirements.

  • Excellent written and verbal communication skills with the ability to build positive relationships with customers, families and stakeholders.

  • Demonstrated commitment to continuous improvement, professional development and person-centred care.

  • Strong clinical decision-making, problem-solving and workload management skills.

A Valuable Addition, Yet Not a Must-Have!

  • Previous experience within aged care, community care or home care settings.

  • Experience in specialty areas including wound management, palliative care, chronic disease management, infection prevention and control, or pain management.

  • Knowledge of the Support at Home or Home Care Package environment.

  • Current Driver’s Licence and ability to work between Chermside and Beaudesert

Essential Requirements

  • Proof of COVID-19 vaccination status (or as required by legislation and WMQ policy).

  • Relevant probity checks required by legislation and WMQ Limited policy, including Aged Care and NDIS screening requirements where applicable.

  • Current Driver’s Licence and willingness to travel for work across the Chermside to Beaudesert region.

  • Current AHPRA registration as a Registered Nurse.
