Remote Patient Monitoring (RPM) Care Coordinator
Our client is a US-based medical practice specializing in sleep medicine. CPAP therapy only works when patients actually use it, and many struggle in silence. The practice runs a remote monitoring program that shows, night by night, who is using their equipment, whose mask is leaking and whose numbers are moving the wrong way. They need a coordinator who turns that data into the right phone call at the right time.
As the RPM Care Coordinator, you will review patient data daily, call patients who need encouragement or troubleshooting, document every interaction accurately, and work with the clinical, equipment, and scheduling teams so patients stay on therapy and progress well. It is a clinically adjacent role: you are the bridge between the data and the patient, and the provider is always one message away when something needs clinical judgment.
Full training on the practice's internal workflows is provided. The practice would like its new coordinator ready to contribute by November 1, 2026.
Key Responsibilities
Daily Data Review and Patient Flagging
- Review patients' CPAP usage data in the remote monitoring platform daily, including usage hours, mask leak rates, AHI scores, and missed nights.
- Identify patients who fall below therapy thresholds or show concerning trends, and prioritize them for outreach.
- Download compliance reports, upload them to the appropriate patient chart in the EHR, and assign them to the correct provider for review.
Patient Outreach and Coaching
- Conduct outbound calls to patients with low usage, elevated mask leaks, or other therapy concerns, using established call scripts and intervention protocols.
- Troubleshoot common CPAP issues, including mask fit problems, pressure discomfort, dry mouth, and air leaks, and guide patients toward solutions.
- Encourage and motivate patients to remain on therapy in a warm, supportive manner—the goal is to help patients succeed, not to pressure them.
- Handle a daily outbound call volume of approximately 60 patient calls.
- Schedule follow-up calls and ensure no patient is lost to follow-up.
Documentation and Billing Support
- Use the monitoring platform's call timer to record monitoring time accurately for each patient interaction, in line with remote patient monitoring billing requirements.
- Document every call in both the monitoring platform and the EHR using standard clinical templates, keeping records complete, timely, and accurate.
- Respond to patient messages in the secure messaging platform within established response-time standards.
Care Coordination
- Coordinate with the DME and CPAP supply team on mask refits, equipment issues, and supply needs identified during patient calls.
- Coordinate with scheduling for follow-up provider appointments when clinically indicated.
- Escalate clinical concerns to the provider promptly and in accordance with established protocols.
Requirements
- Minimum 1 year of experience in remote patient monitoring, chronic care management, or patient outreach—ideally with sleep medicine, CPAP therapy, or DME patients.
- Solid understanding of CPAP therapy, including common patient challenges such as mask fit issues, pressure intolerance, dry mouth, and air leaks.
- A warm, patient, and encouraging phone presence—you know how to motivate a patient who is struggling without making them feel pressured or judged.
- Organized and detail-oriented, with a consistent habit of accurate and timely documentation.
- Comfortable reviewing data dashboards and identifying patients who need follow-up based on usage trends.
- Strong understanding of HIPAA and patient confidentiality requirements, particularly in a remote work environment.
- Reliable high-speed internet connection and a quiet, private workspace suitable for clinical patient calls.
Preferred Qualifications
- Clinical background such as a nursing degree (RN or LPN), medical assistant certification, respiratory therapist (RT) credential, or equivalent.
- Experience with Sanusom, Athena (athenahealth), or Klara.
- Prior experience in a sleep medicine practice, DME company, or CPAP compliance program.
- Familiarity with remote patient monitoring (RPM) and remote therapeutic monitoring (RTM) billing codes and documentation requirements.
Tools & Software
- Sanusom—remote monitoring platform for CPAP usage data, prioritization, call timer, and encounter notes.
- Athena (athenahealth)—electronic health record for charts, documents, and provider review.
- Klara—secure two-way patient messaging.
- A practice-provided phone or softphone for outbound patient calls.
- Microsoft Teams and OneDrive for day-to-day work with Remote Raven.
What Makes You a Great Fit
- You see that someone has not been using their equipment, and you pick up the phone—because you know many patients struggle in silence.
- Patients feel heard when you call. You ask before you advise, and you leave them with practical help, not a checklist.
- You can manage a high daily call volume without losing track of anyone, and every patient on your list has a next step.
- You are disciplined about documentation: every interaction recorded correctly, every minute logged as it happened.
- You know the difference between a coaching call and a clinical concern, and you get the provider involved the same day when it is the second.
